After engaging with the learning resources in this module, your knowledge and understanding of the importance of “Teams” for achieving optimum outcomes has grown.

Now it is time to examine the Team structure and Team process in your practice setting and to identify the essential Team members for promoting the success of your proposed project.

Create a presentation that provides the following information:

    1.  An overview of the concept of the value of Teams in enhancing patient quality and safety, with references. (anticipated 1 to 2 slides)

    2.  The structure of the Team in your practice setting.

    ·

    · Construct a diagram/flow chart on a slide that depicts the Team structure in your practice setting that is relevant to your proposed student project (you are encouraged to be comprehensive in developing this aspect of the assignment)

    3. Next you will construct a ‘Lean Process Map’ that delineates the current Team process relevant to the problem you have identified as the focus for graduate student project

    ·

    · Create a slide that provides a brief overview of the identified ‘practice problem’ your graduate student project will be designed to address

    · Create a slide that demonstrates a ‘Process Map’ that outlines the current Team process relevant to the identified practice problem [review this week’s video resource “Integrating Quality Improvement Methods to Improve Teamwork and Patient Safety (TeamSTEPPS)” minute marker 10:04 to 12:00 for a demonstration of what a process map is].

    4.  Answer the following question – Do you anticipate the Team structure and processes you have identified will be a barrier to implementing your project, OR will they provide a foundation for success of implementing your proposed project? Provide a detailed rationale for your answer.

    5.  Now that you have charted out and examined the existing Team structure and processes relevant to your proposed graduate student project complete the following:

    · Critically examine and reflect on the initial diagram/flow chart you created earlier in the presentation for the Team structure in your practice setting. Now that you have created the ‘Process Map’ – Do you believe the Team structure you diagrammed accurately reflects the Team structure? Yes or No? Create a slide with your answer and provide an explanation that supports it.

    · Construct a slide that demonstrates a ‘Process Map’ that reflects the process that you believe best supports, and is aligned with, your proposed graduate student project.

Health Problem Proposal: Hypertension and Health Disparities

Health Promotion & Role Development in Adv. Nursing Practice

    Dr. Nora Hernandez-Pupo

    November 8, 2025

Health Problem Proposal: Hypertension and Health Disparities

Hypertension is a widespread and extremely important public health problem given its

wide-ranging impact on cardiovascular morbidity and mortality. Hypertension can be

asymptomatic for many individuals, yet it plays a significant role in heart disease, stroke, kidney

failure, and other critical health conditions. Given its high prevalence and the fact that control is

still suboptimal, hypertension is a major burden to health systems and individual quality of life

(Goorani et al. 2024). Importantly, differences are present in the occurrence, recognition,

management, and regulation across racial/ethnic, socioeconomic, and geographic boundaries,

emphasizing the necessity for health promotion and prevention initiatives to address both clinical

health aspects and social determinants of health. The substantial health impact of hypertension

provides a rationale for the choice of this condition for implementation in a health promotion

program that addresses the issues of disparities at multiple levels.

    Global Relevance

Globally, hypertension is becoming a major global burden of disease-causing millions of

deaths per year associated with cardiovascular disease. Although the occurrence of uncontrolled

BP differs globally and regionally, in low and middle-income countries, a growing burden of

uncontrolled BP is associated with low access to care and resources (Schutte et al., 2021).

Almost half of hypertensive adults worldwide are unaware of their condition, elevating their risk.

(CDC, 2025). This is a global issue, which contributes to the underdevelopment of multinational

and cross-culture strategies for blood pressure awareness and control. International screening,

education and standardized treatment protocols are important because hypertension is usually

"silent." The international nature of hypertension makes it clear that while the previous efforts to

    3

address it through local interventions are very important, the international nature of the problem

    requires coordination across health systems.

    National Level

At the national level in the US, however, the prevalence of hypertension has remained

alarmingly high. According to data available for August 2021-August 2023, nearly 1 in 2 US

adults had hypertension (defined as >=130/80 mm Hg or on antihypertensive medication) at 47.7

percent (Fryar et al., 2024). The rates of awareness, treatment, and control remain below optimal

levels. For instance, only one-fifth of adults with hypertension achieved blood pressure control

below 130/80 mm Hg during the specified period. Racial and ethnic disparities persist: non-

Hispanic Black individuals experience higher rates of hypertension compared to non-Hispanic

White or Hispanic individuals (48.8 percent versus 37.6 percent and 27.9 percent, respectively)

and face challenges with controlling blood pressure (Sells et al., 2023). These disparities are

linked to socioeconomic status and educational accomplishment, and access to care. For

example, poverty and lower education are associated with cardiovascular disease, particularly in

Black and Hispanic adults. Given the prevalence, poor control, and inequities in control and

treatment, a national health promotion strategy targeting understanding, screening, access, and

    control is clearly needed.

    Local Level

At a more localized level, within the state of Florida, hypertension continues to pose a

significant concern with spatial and social segregation. A recent data set shows that 35.9 percent

of Florida adults reported having been told by a health professional that they had high BP.

(America’s Health Ranking, 2023). Among federally qualified health centers (FQHCs) in Florida

    4

in 2017, the statewide average hypertension control rate was about 57 % with much lower

hypertension control rates (37.8 %-51.4 %) in the lowest tertile, occurring mostly in northern

Florida counties which also have higher rates of BP-related death (CDC, 2024). These data

characterize the communities of disparity within a single state: factors of geography,

socioeconomic, and access are at play in creating disparities. For a health-promotion endeavor in

Florida, targeting areas with low control rates as well as underserved populations and/or linkage

to primary care and community outreach would be directly relevant and warranted.

    Conclusion

Given the widespread prevalence of hypertension around the world, as well as the high

prevalence of the condition in the United States as a whole, as well as the differences in

prevalence, control, and outcomes that are specific to the state of Florida, the argument in favor

of developing multilevel health promotion programs is persuasive. For programs to be

successful, they must be implemented on a global, national, and local scale. Several of the

promotion strategies have the potential to lessen the burden of hypertension and to reduce health

disparities. These strategies include addressing the social determinants of health, developing

greater access to screening and care, improving awareness and compliance, and tailoring the

intervention to the needs of populations that are underserved. Therefore, hypertension is a health

problem selection that is suitable for the construction of a health promotion program that will

span the global, national, and local levels, as well as the inequities that exist in the prevention

and control of the problem. Both of these levels will be addressed in the program.

Data to support patient care comes from a variety of sources that contain differing data types. Key activities to use clinical data include identifying the sources of data, understanding the data types and associated methods to work with the data, and identifying the necessary resources to complete your IT project.
The scope of your IT project will determine the level of data access required and the associated data storage needs. Data used in multisite projects will require IRB oversight and often require the execution of a DUA if transferring data outside of the institution or receiving data from another institution.
Identifying and assembling an adequate project team is based on the needs of the project. At a minimum, you will need to include frontline staff that will use the product, a data analyst capable of completing the ETL process on the data, and potentially statisticians to conduct appropriate model building and outcomes analyses.
There are multiple approaches to analyzing data. AI is the latest advance in machine learning approaches that include supervised, in which data is labeled and the algorithm is guided with statistical considerations, and unsupervised, in which unlabeled data is used to infer meaning. While robust, machine learning approaches require interdisciplinary teams and large resource dedication to complete.
All projects require review and potential revision over time. Follow-up and review of implemented programs should be included in the initial planning stages and resource allocation decisions at project inception.

Addressing Obesity as a Global, National, and Local Health Problem

    Overview of the Health Problem

Obesity is currently among the most significant social health issues of the 21 st century. It

is a complex condition which is characterized by accumulation of excess fat within the body

which is hazardous to the health of the individual as it might result to cardiovascular disease,

type 2 diabetes, stroke and certain forms of cancer. Obesity is an ailment that is defined by

World Health Organization (WHO) as a body mass index (BMI) of 30kg / m2 or above as an

indicator of an unhealthy equilibrium of the caloric intake and the expenditure of energy (WHO,

2020). The health outcomes of obesity are not individual outcomes, but they are stressors to

health care systems, lower productivity and cause the social stigma and discrimination. The

problem of obesity is preventable through change of lifestyles, but there has been a continued

trend in the severity of obesity throughout the entire world due to the unhealthy dietary choices,

a lack of physical exercise, the existence of socioeconomic inequalities that obstruct access to

    nutritious food and recreational spaces.

    Global Relevance

Obesity has already become epidemic in the entire world, and its manifestation is

observed in all age groups and all socioeconomic classes. The World Health Organization (2020)

has estimated that over 1 billion individuals across the world are living with obesity of 650

million adults, 340 million adolescents, and 39 million children. The prevalence of obesity in the

world has almost tripled in the world since 1975, because of both the increased access to high

calories foods that lack nutrients as well as the reduced rates of physical activity (World Health

Organization, 2025). The developing countries today are facing the so-called double burden of

malnutrition, in which undernutrition is accompanied by the increasing incidence of obesity. In

    3

countries where urbanization is at a high rate like India, Brazil, and Mexico, poor eating habits,

physical inactivity are the dominant causes of obesity-associated illnesses (Ahmed and

Mohammed, 2025). The economic burden of obesity is estimated at more than 2 trillion in terms

of global economy every year in the world, which is about 2.8 percent of the gross domestic

product (Aremu et al., 2025). These statistics highlight the pressing need of obesity to be tackled

through concerted international preventive measures that have put the importance of nutrition

education, change in food policy and community-based health promotion.

    National Relevance

Obesity is a serious health issue facing the people of the United States because more than

43 percent of adults and 16 percent of children and adolescents are obese (Ng et al., 2025). The

number has been rising consistently across the demographics, with Non-Hispanic Black (49.9)

and Hispanic adults having the highest rates of 45.6 (Herren et al., 2022). Obesity risk is greatly

determined by socioeconomic factors such as income level, education and access to healthcare.

Lots of communities that have low-income deal with so-called food deserts, where there is a low

level of access to healthy food, and food swamps, where unhealthy food is more prevalent. In the

U.S., the economic cost of obesity is more than 173 billion USD in direct medical expenditure

and obese people spend 1861 USD more per year in medical care than people of normal weight

(Cawley, 2021). Healthy People 2030 (2024) says that the prevalence of obesity is to be

mitigated by national efforts like the U.S. Department of Health and Human Services' efforts in

its initiative, titled Healthy People 2030, and the CDC's undertaking, titled State and Local

    Programs to Prevent Obesity.

    4

    Local Relevance

Obesity is a major issue in most communities in the U.S, especially the one with poverty

and limited health opportunities. As an illustration, disparities even exist in California where

there is a wide range of efforts in the field of public health. The (O'Lawrence et al., 2020) report

that around 29 percent of adults and 17 percent of children are obese, and the prevalence is

higher among the low-income and minority groups. The proportion of adults who say they are

obese in Los Angeles County alone is over 25 percent, and one out of every five children has

become overweight or obese (Ahmed & Mohammed, 2025). The local factors influencing these

trends are high cost of life that limits consumption of healthy foods, absence of recreational

centers and cultural eating habits. Community based interventions such as school lunch,

community exercise and city initiative to promote consumption of fresh produce such as farmers

markets have depicted promising outcomes. However, obesity is still a problem among

underserved communities, so health promotion efforts still have to be implemented, and the local

    background should be considered.

    Justification for Health Promotion Strategies

The additional proliferation of obesity on global, national and local levels testified to the

need of multisided strategies of health promotion. With regards to obesity interdisciplinary

collaboration between healthcare practitioners, policymakers, schools and communities is

necessary to enable behavior change, healthy environment and socioeconomic barriers to

wellness. The health promotion programs must be provided on prevention, culturally sensitive

education and equally access to healthier foods and greater access to physical activities. The

prevalence of obesity and improvement of the quality of life in societies can be prevented by

means of certain interventions such as dietary education, physical activity, and changes in

    5

policies. Obesity epidemics should be reversed by initiating wide-ranging health promotion

programs that would guarantee a cleaner and more balanced future.
